Course Description: (Flyer)

“Transitions of Care” refers to the movement of patients between health care practitioners, settings, and home as their condition and care needs change. Ineffective care transition processes lead to adverse events and higher hospital readmission rates and costs. Join us to learn the various levels of care and how nurses impact care coordination outcomes.

Accreditation:

Please allow ample time for traffic and parking. No partial credit given.

Nurses will be awarded 6.5 ANCC contact hours for participation in the course.

UCLA Health is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

Provider approved by the California Board of Registered Nursing, Provider number 12511 for 8 hours.

Target Audience:

This course is offered to UCLA Medical Center, Home health agencies, SNFs and STTI Gamma Tau Member: nurses (including RNs & LVNs), nurse practitioners, physicians & physician assistants, Care Partners (CPs), EMT/paramedics, ACCPs, and Mental Health Professionals

Deadline for registration:

Online registration ends on Monday, December 7.

Driving Directions:

From the San Diego Freeway (405):
From the north, exit Wilshire East; or from the south, exit Wilshire Westwood. Turn left on Westwood Blvd. and go straight past Le Conte Avenue (the street becomes Westwood Plaza). The hospital is on the left, past UCLA Medical Plaza.

From Los Angeles International Airport (LAX):
Take the San Diego Freeway northbound to Wilshire Boulevard east, and continue as described above.

From the East via the 10 (Santa Monica Freeway):
Take 10 (Santa Monica Fwy) East to 405 (San Diego Fwy) North, and exit on Wilshire Boulevard east. Continue as described above.
